Any registrant whose registration has expired for a period exceeding one (1) year may apply for reinstatement via the online eLicense portal. You may request step-by-step reinstatement instructions via email at firstname.lastname@example.org. Please include your registration name and license number.
In addition to the online questions, the following will need to be uploaded online as part of your application:
- A detailed statement covering all experience subsequent to the date of expiration. This statement shall describe the nature of work performed, degree of responsibility, and dates of each engagement.
- A written statement explaining the reason for terminating active registration and the reason for requesting reinstatement.
- A completed continuing professional development (CPD) log summarizing the CPD hours submitted.
- Certificates of attendance verifying completion of the CPD requirement for all lapsed years (up to 3 years or 45 hours)
If your license has been expired four (4) years or more in the state of Ohio, please read the following:
Pursuant to Ohio Administrative Code Sections 4733-19-01 and 4733-25-01, a registrant whose registration has expired more than four (4) consecutive years shall be required to take and pass the Principles and Practice of Engineering and/or Surveying examination as a condition of reinstatement unless the registrant has been continuously licensed in another jurisdiction within the United States.
If your license has been lapsed in Ohio for 4 years or more but you have maintained continuous licensure in another U.S. state or territory, you must have the Reinstatement Information Request Form (4013) completed by the Licensure Board in the state(s) where you held continuous licensure. The verifying Licensure Board will complete this form and send the information directly to us. If your license has been inactive for less than 4 years in Ohio at the time of your application for reinstatement, you do not need to have this form completed.
