Individual Address Change

  • FYI…When contacting the Board by email, do not use ALL CAPS in the subject header. Our email filter interprets ALL CAPS as junk mail and we may not receive your communication.

You are responsible for keeping your address current. The Board requires written verification of an address change.

The easiest way to update your address is to email the Board with the change. Include “Address change” in the subject header. You can also fax or mail your change.

The Board cannot accept address changes by telephone; we need written or printed verification of your address change.

Individual Name Change

Most frequently name changes include firms with a Certificate of Authorization. On occasion, name changes include individuals.

The easiest way to update your individual name change is to email the Board with the change. Include “Name change” in the subject header. You can also fax or mail your change. Be sure to include accompanying documentation with proof of name change.

The Board cannot accept name changes by telephone; we need written or printed verification of your individual name change.

Company Address Change

Company Name Change

A company name change requires a new application in order to maintain a Certificate of Authorization. The Board cannot accept a company name change other than through a new application. For Certificate of Authorization information, go to our Companies page.
