Overview
Any firm providing, or offering to provide, engineering or surveying services in the state of Ohio needs a Certificate of Authorization from the State Board of Registration for Professional Engineers and Surveyors. You can find the legal requirements in Ohio R.C. section 4733.16 and Ohio A.C. section 4733-39.
The Certificate of Authorization application requires that names and addresses of all owners of the firm be included with the application — a list of officers will not suffice. If an owner is an officer, please state that. In addition, if the firm is owned by another corporation, you must list the ownership of that firm. For example: “ABC LCC is owned by ABC, Inc. which is a publicly traded firm,” or list the owners of said firm.
If the firm is employee-owned, you must state that and include the names and addresses of the individual owners. If the ownership information is not included, the application will be returned as incomplete.
Firm Annual Renewal Application Packet
IMPORTANT! The Board will only accept PES 3011 and PES 3012 (Rev. 04/2009 and later) for Certificate of Authorization renewal applications. Older application forms received will be returned.
The renewal fee is $25.00 on or before June 30 and $37.50 on or after July 1.
The Board mails firm Certificate of Authorization annual renewal reminders each April. Certificates of Authorization to provide engineering and surveying services in the state of Ohio expire each year on June 30.
Please note that the renewal form requires your firm’s 5-digit Certificate of Authorization license number which you can find near the upper center of your renewal reminder, on your current license, or from our License Lookup page.
Firm New or Name Change Application Packet
IMPORTANT! The Board will only accept PES 3011 and PES 3012 (Rev. 04/2009 and later) for new or name change Certificate of Authorization applications. Older application forms received will be returned.
The new or name change application fee is $50.00.
Instructions are included with the application packet.
“Anyone who is planning to do business within the state of Ohio, using a name other than their own personal name, must register with our office. Sole proprietorships are not required to register with the Secretary of State’s office. This applies to companies in Ohio and companies in other states or countries wishing to do business in the state of Ohio. Companies outside the state of Ohio often have to include a Certificate of Good Standing from their home state’s Secretary of State or registering authority to accompany their filing.”
