Individual Address Change
You are responsible for keeping your address current. The Board requires written verification of an address change.
The easiest way to update your address is to email the Board with the change. Include “Address change” in the subject header. You can also fax or mail your change.
The Board cannot accept address changes by telephone; we need written or printed verification of your address change.
Individual Name Change
Most frequently name changes include firms with a Certificate of Authorization. On occasion, name changes include individuals.
The easiest way to update your individual name change is to email the Board with the change. Include “Name change” in the subject header. You can also fax or mail your change.

Firm Address Change

Firm Name Change
